Community Support Program (CSP)
The Community Support Program (CSP) is a short-term program that provides intensive case management services to individuals with long-standing mental health or substance abuse issues. CSP clients typically have a history of previous inpatient psychiatric hospitalizations or episodes of impairment that place them at risk for hospitalization including a pattern of non-compliance with outpatient behavioral health services. The Community Support Program (CSP) is a voluntary program that offers time limited services to individuals with mental health or substance abuse issues. The average length of stay in the program is three to six months. CSP provides services to adults, adolescents, and children. All services are based on the individual and are solution-focused. The Community Support Program’s services are intended to complement other services that are - or - will be put in place for clients. Services are delivered in the community by a team of mobile Community Support Providers. The CSP model is designed to be maximally flexible in supporting clients’ access to and sustained involvement with necessary treatment and resources. CSP promotes a long-term community linkage for clients, in order to reduce the amount of time spent at an inpatient level of care. Community Support Program Services Include: Case Management: Assist consumers with coordination of care and treatment planning while forming, establishing and maintaining a positive alliance and a supportive working relationship. Symptom Management: Assist consumers with understanding the nature of their illness or addiction, recognizing symptoms and how medication compliance may play a role in their recovery. Conflict Resolution: Develop a mechanism to assist consumers with the resolution of interpersonal conflicts. Daily Living Skills: Assist consumers in enhancing daily living skills. Socialization/Leisure Skills: Assist consumers with the development of interpersonal skills and the research of recreational and self-help opportunities. Service Coordination: Work with the consumer and all providers to ensure an integrated treatment planning process. Resource Management and Referrals: Assist the individual in obtaining entitlements, health, social, and community services. This can include taking clients to necessary appointments at state agencies, helping clients with filling out applications for benefits or advocating for appeals, and acting as a central point of reference for the entire treatment team. Crisis Intervention: Assist consumers in developing a detailed crisis plan and connecting them with the Boston Emergency Services Team (BEST) for around-the-clock support.
